DIRECTIONS FOR COMPLETING SUPPLEMENTAL APPLICATION

A completed supplemental application is required of all applicants to the University of Iowa College of Pharmacy in addition to the PharmCAS application.

Download either the PDF or Word file by clicking on the appropriate link below and saving the file to your local drive. For either file type, you may choose to type the requested information directly into the interactive form fields within the supplemental application and then print the document (strongly preferred), or you may print the document first then enter the requested information by hand, printing neatly in blue or black ink. Please note you only need to complete and mail ONE supplemental application and supplemental application fee. However, we are providing the application in two file formats for your convenience.

MAILING INSTRUCTIONS

Make sure to sign and date the completed supplemental application before mailing. Mail the completed supplemental application, along with a $100.00 US non-refundable supplemental application fee in the form of a check or money order made payable to the University of Iowa College of Pharmacy, to:

DO NOT SEND CASH. Credit cards are not accepted. Supplemental applications must be postmarked by December 1, 2009. Make sure to retain a copy for your records. FEE WAIVER POLICY

If a candidate is unable to pay the Supplemental Application fee because of economic hardship, the candidate may apply for a fee waiver. To apply for a fee waiver, the candidate should request that a letter be sent to the above address from the Office of Student Financial Aid (or equivalent) at their current college or university substantiating that paying the fee would be an economic hardship to the candidate. This letter should include information regarding the student's Expected Family Contribution (EFC) for the current school year.  Applicants who received a fee waiver from PharmCAS will be eligible to receive a supplemental application fee waiver without supplying additional information.  Applicants who applied for a fee waiver from PharmCAS and were notified they were eligible for a PharmCAS fee waiver, but did not recieve the waiver due to limited availability, will also be eligible to receive a supplemental application fee waiver without supplying additional information.  If you have any questions, contact the Office of Academic Affairs at (319) 335-8795 or toll-free (800) 553-IOWA, ext 5-8795, or email pharmacy-academic-affairs@uiowa.edu.
