Matthew A. Cantrell

Clinical Practice
Dr. Cantrell works in several ambulatory care settings in the Veterans Affairs health care system including primary care, a pharmacist managed anticoagulation clinic, and a pharmacist-managed lipid clinic consisting mainly of high cardiovascular risk patients. The anticoagulation clinic providers at the VA which currently manages over 1,200 patients was recently recognized as a "Best Practices Award" at the most recent Joint Commission Survey.  Dr. Cantrell also serves as a preceptor for the VA primary care residency program of which he completed in 2006. He is an active member of state and local pharmacy organizations as well as the National Lipid Association.  Current research interests include urology, documentation of cognitive pharmacy services, and teaching assessment methods. In addition to serving a rotation site for fourth year pharmacy students he particularly enjoys interacting with students in Pharmacy Practice Lab and discussing patient care related topics with students in the IPE I and IPE III programs.

Clinical Practice
Dr. Ernst’s clinical practice interests are in the area of infectious diseases and appropriate use of antimicrobial agents.  Her clinical practice site is the University of Iowa Hospitals and Clinics infectious diseases consult service.  While on service she participates in daily medical rounds with the infectious diseases consult service, providing drug information and drug therapy evaluation to physicians, residents, and medical and pharmacy students.  She is also a member of the pharmacy and therapeutics committee and the antibiotic advisory subcommittee at UIHC.   Her interests include appropriate use of antimicrobial agents, pharmacoepidemiology and antimicrobial resistance.  Please see the faculty research directory (http://pharmacy.uiowa.edu/grad/profiles.htm) for a description of her research program.

Clinical Practice
Dr. Ernst is a clinical pharmacist and Associate Professor (Clinical) in the College of Pharmacy, with a secondary appointment in the Department of Family Medicine, Carver College of Medicine His practice areas within the Family Medicine clinic encompass both the outpatient and inpatient settings where he is engaged in direct patient care, as well as traditional provision of drug information and pharmacotherapy consultation to physicians and allied health care providers in the clinic.  He maintains active practice and research interests in the areas of hypertension and geriatric patient care.  He founded and directs the Ambulatory Blood Pressure Monitoring referral service within the Family Care Center.  He is an active participant in the pharmacist-directed anticoagulation case management service, as well as the multi-disciplinary geriatric assessment clinic.  As a faculty member of the Department, he participates directly in the education of medical residents and allied health professional students in the Family Medicine Residency Program.

In addition to his patient care responsibilities in the Department of Family Medicine in Iowa City, Dr. Ernst is also a consultant clinical pharmacist at the Iowa Health Physicians Mt. Vernon Family Practice Clinic in Mt. Vernon, Iowa, where he provides outpatient anticoagulation services one day per week.

Dr. Ernst has published over 50 scholarly review articles, research articles, book chapters, and monographs encompassing a broad array of topics commonly encountered in the primary care setting.

Clinical Practice
The Iowa Drug Information Network (IDIN) is an innovative program developed by the Division of Drug Information Service of the College of Pharmacy in collaboration with faculty in the Clinical and Administrative Division, at the Family Practice sites, and at community pharmaceutical care practice sites. IDIN provides access via the internet to drug information databases and electronic communication links to the pharmaceutical care practice and teaching sites, and personal consultation for patient specific medication information.    

IDIN supports the drug information role of pharmacists providing pharmaceutical care throughout the state. It is a center designed for the generation and communication of medication information to preceptor sites.  In addition, IDIN provides drug information and evaluation services on a fee basis to home healthcare groups, chain pharmacies and others.

IDIN responds to information requests which come from pharmacists 69% of the time, students and residents 21% and other health professionals 9%.  The types of questions asked include: adverse drug reactions or drug interactions 14%, therapeutic use 34%, product availability or identification 2%, drug dosage 9%, pharmaceutical compatibility/stability 12%, therapeutic drug monitoring 2%, formulary reviews 3%, and other requests 24%.  All drug information requests are recorded in a database where relevant responses (currently over 1000 records) can be peer-reviewed for accuracy and completeness and they are available to any IDIS/Web subscriber. 

Residents and students gain practical experience in providing drug information and develop skills in (1) perceiving, assessing and evaluating the medication information needs of health care professionals, and (2) using a systematic approach for effectively searching, retrieving, evaluating, communicating and applying data from the published literature and other sources applicable to the patient care situation.  They gain practical skills to effectively write and present that information.

Kathleen E. Horner

Dr. Horner is an Assistant Professor (Clinical) at the University of Iowa College of Pharmacy and a Clinical Pharmacy Specialist in Ambulatory Care at the University of Iowa Hospitals and Clinics.  Her primary practices sites include the Family Care Center General Internal Medicine Clinic, Pharmaceutical Case Management Service, Anticoagulation Case Management Service, and Geriatrics Clinic.  She serves as a preceptor for pharmacy residents and student pharmacists in these clinical areas.  In addition to her areas of practice, her other interests include management of hypertension, hyperlipidemia, and diabetes.

Kristin Horning

Clinical Practice
Dr. Horning practices at the East Des Moines Family Care Center, which is a teaching clinic for 16 family medicine physician residents.  In addition, the clinic site has 6 faculty physicians and an OB/GYN.  The East Des Moines Family Care Center provides a variety of ambulatory patient services.  Dr. Horning’s clinic services include anticoagulation clinic, diabetes education, smoking cessation, asthma education, and hypertension management.  She also attends inpatient rounds at Iowa Lutheran Hospital with the residency service.  Iowa Lutheran Hospital is a private, community hospital which services a diverse patient population.  She serves as a preceptor for pharmacy students completing a family medicine rotation and incorporates them into all areas of her practice.  Dr. Horning’s professional interests include cardiovascular disease, diabetes, and chronic disease state management.

Dr. Johnson’s clinical practice is Antimicrobial Management at UIHC.  The Antimicrobial Management program is responsible for development and implementation of guidelines and protocols for appropriate antimicrobial use at UIHC.  Education to health care professionals regarding antimicrobial use is also carried out through the program.  Dr. Johnson is responsible for monitoring patients on antibiotic therapy and making interventions, when appropriate, in conjunction with an Infectious Diseases physician.

Dr. McDanel is a Clinical Pharmacy Specialist in Ambulatory Care at the University of Iowa Hospitals and Clinics and Assistant Professor (Clinical) at the University of Iowa College of Pharmacy. She also is the new Program Director of the Ambulatory Care Pharmacy Residency at UIHC.  Her primary practice sites include the Family Care Center Internal Medicine Clinic, Pharmaceutical Case Management Service, Anticoagulation Case Management Service, and the Allergy/Immunology Clinic.  She serves as a preceptor to both residents and Pharm.D. students in these areas.  She received her Doctorate of Pharmacy degree from the University of Iowa in 2001 and completed an ASHP-accredited Specialized Residency in Primary Care at the University of Iowa Hospitals and Clinics in 2002.  In addition to her areas of practice, her other interests include hypertension, hyperlipidemia, and diabetes.

Erin Newkirk, PharmD, BCPS

Dr. Newkirk is a Clinical Pharmacy Specialist in Ambulatory Care at the University of Iowa Hospital and Clinics and Assistant Professor (Clinical) at the University of Iowa College of Pharmacy.  She is also the Director of the Anticoagulation Case Management Service. Her primary practice sites include Anticoagulation, Diabetes Management, Smoking Cessation, Internal Medicine, and Pharmaceutical Case Management Clinics.  She serves as a preceptor to both residents and PharmD students in her clinical areas.  Dr. Newkirk received her PharmD degree from the University of Wisconsin and completed an ASHP-accredited Specialized Residency in Primary Care at the William S. Middleton Memorial Veterans Hospital in Madison, Wisconsin.

James A. Ponto

Mr. Ponto is Board Certified in Nuclear Pharmacy and has a primary appointment as Chief Nuclear Pharmacist in the Department of Radiology, Division of Nuclear Medicine in the University of Iowa Hospitals and Clinics.  In his practitioner role, he is responsible for directing the administrative, operational, and professional activities of UIHC’s nuclear pharmacy lab.  Routine tasks include the ordering, receiving, storage, preparation, quality control testing, dispensing, and disposal of radiopharmaceuticals and related drugs and supplies.  He also provides both didactic and experiential instruction to nuclear medicine technology students, radiology and nuclear medicine residents, and others.  He is an active participant in the varied and diverse research efforts relating to radiopharmaceuticals, notably clinical trials.

Mr. Ponto is an active member of professional organizations, notably the American Pharmacists Association, Nuclear Pharmacy Section, the Society of Nuclear Medicine, and the National Association of Nuclear Pharmacies.  He currently serves on the United Stated Pharmacopeia Radiopharmaceuticals Information Expert Committee.  Mr. Ponto has received several honors and awards, including Fellow of the American Pharmaceutical Association, Fellow of the American Society of Hospital Pharmacists, the Distinguished Achievement Award in Nuclear Pharmacy Practice (APhA), and the Daniel B. Smith Practice Excellence Award (APhA).

Dr. Reist is an Assistant Professor (Clinical) and coordinator of the Pharmacy Practice Laboratory at the University of Iowa College of Pharmacy. He practices as a Clinical Pharmacy Specialist in Ambulatory Care at the University of Iowa Hospitals and Clinics in the areas of geriatrics and anticoagulation. Prior to returning to the College to teach full-time, Jeff worked in hospital, community and long-term care pharmacy practices in the Cedar Rapids area and served as adjunct faculty precepting pharmacy students. He is a 1982 graduate of the University of Iowa College of Pharmacy (BS Pharm) and a 2007 graduate of the University of Florida College of Pharmacy (PharmD).  Jeff is a member of the American Pharmacists Association, American Association of Colleges of Pharmacy, American Society of Consultant Pharmacists, and the Iowa Pharmacy Association.

 

Heather R. Bream-Rouwenhorst

Dr. Bream-Rouwenhorst is a Clinical Pharmacy Specialist in Surgery/Transplant at the Veterans Affairs Medical Center and Assistant Professor (Clinical) at the University of Iowa College of Pharmacy.  Dr. Bream-Rouwenhorst actively participates in inpatient rounds, intervening on drug therapy problems, providing drug information, and focusing on providing continuity of care between the inpatient and outpatient setting.  She is also actively involved in medication teaching for solid organ transplant patients.  Dr. Bream-Rouwenhorst serves as a preceptor to pharmacy students for both introductory and advanced clinical practice experiences.   Her clinical interest areas include surgery, transplant, critical care, infectious disease, cardiology, and internal medicine.   She received her Doctorate of Pharmacy from the University of Iowa and completed a General Practice Residency and a Specialty Residency in Critical Care at the University of Iowa Hospitals and Clinics.

Dr. Shaw is a Clinical Pharmacy Specialist in Critical Care and Nutrition Support at the Iowa CityVeterans Affairs Medical Center and Assistant Professor (Clinical) at the University of Iowa College of Pharmacy.  He participates in the Intensive Care Unit interdisciplinary and internal medicine inpatient rounds. He is also a member of the Nutrition Support Team providing specialized nutrition support to inpatients. Dr. Shaw serves as a preceptor to pharmacy students for acute care medicine practice experiences.   His clinical interest areas include critical care, internal medicine and specialized nutrition support.   He received his Bachelors of Science in Pharmacy and Doctorate of Pharmacy from the University of Illinois, and a Masters in Public Health from the University of South Florida. Dr. Shaw completed a Specialty Residency in Critical Care at Tampa General Hospital in Tampa, Florida. In addition to his areas of practice, professional interests include statistics and health care data.

 

SwegleJohn M. Swegle, B.S., Pharm.D., BCPS

Clinical Practice
Dr. Swegle is a faculty member and clinical pharmacist for the Mercy Family Medicine Residency in Mason City.  The Family Medicine residency is a training program for 18 residents and is affiliated with Mercy Medical Center – North Iowa.  The residency program serves as a teaching center for students of various disciplines including medicine, nursing, and pharmacy.  His practice area focuses on drug therapy management for patients in the hospital, ambulatory, and nursing home settings.   Dr Swegle serves as a preceptor for pharmacy students and pharmacy practice residents. 

In addition to teaching responsibilities, he serves as a member of the Pharmacy and Therapeutics Subcommittee and the Institutional Review Board for Mercy Medical Center – North Iowa.  He also collaborates with the Palliative Medicine team and serves as a clinical pharmacist for Hospice of North Iowa.

His professional interests include geriatrics, pain management, chronic disease state management, and end-of-life care.

Dr. Trewet’s clinical site is in Des Moines, Iowa at Broadlawns Family Health Center in coordination with a family medicine physician residency training program.  The Broadlawns Medical Center Family Practice Residency Program encompasses numerous clinics and outpatient services.  Dr. Trewet actively participates in the inpatient rounding service and focuses on providing continuity of care between the inpatient and outpatient setting.  Her ambulatory care services include anticoagulation, diabetes, smoking cessation and cardiovascular risk clinics.  Her clinical interest areas include preventative health, cardiology, diabetes, COPD, women’s health, depression and continuing professional development.  Dr. Trewet integrates students on rotation with her into all areas at her site allowing them to experience firsthand physician and pharmacist collaboration.  She enjoys the challenges that students and patients bring her way every day.
